of Interrogatories (1996) [case number: CV 191-CC-2880]
filed in Greene County Court House in Springfield, Missouri.
This is not an exact copy; a number of amendments and
insertions have been made in order to enhance clarity or
protect the privacy of others.
In the last five or so years, Marie has received no federal or
state, community, or church assistance.
[Marie did, however, receive a $25 cash donation from a Jewish
rabbi several years
ago.]
Question
5.
State all types of government and/or public assistance
from which you have received benefits, financial or otherwise in
the past five (5) years.
For each type of assistance state:

I have record of assistance provided on or about November or
December 1993, May 12, 1994, June 24, 1994, and July 7, 1994.
Although I believe there may be one or two additional incidents in
1993 or 1994, I do not recall the dates and do not have record of
them.
I did not receive any monetary food assistance from the Polk County
Community Center following this and did not obtain any actual food
from the Polk Community Food Bank because of my specialized
organic diet.
Additionally, I received no Food Stamps after I was accepted for
Social Security Disability Insurance because Ethel
May Tennis, my
Food Stamp case worker, alerted my representative that she believed
that the Director of the Polk County Division
of Family Services,
Terry Phillips
(an instructor and prominent member of the Sociology
and Psychology Department at SBU) would demand, by force if
necessary, an "in-home evaluation" if I were to request any additional
aid.
As Mrs. Tennis had warned, shortly
thereafter, in spite of my doctor's
clear instruction that doing so would endanger my health and
life, the
electricity to my home was disconnected. My representative's
call for
assistance to Polk County Family Services (inquiring about the
Energy
Assistance Program and Project Help) precipitated the forced
entry
and other events of September and October 1992 as described
briefly
in the
First
Set of Interrogatories, question 31, page 369-373.

I received fifty dollars ($50) on each of these four occasions.
I am uncertain if any of these funds actually came from the
Polk County Community Center or if these funds were part
of an anonymous donation which was funneled through the
Center.
For several years, my representative was told by the Director
of the Polk County Community Center, Mrs.
Ruby Payne, that
I was not deserving of assistance because the Center's limited
funds were to be used primarily for families with children and
not for the disabled.
Mrs. Payne did allow several small
anonymous contributions
(as detailed in this Interrogatory) to be given through the
Center.
However, in the Spring of 1993, I believe,
Ms. Payne informed
my representative that the Polk County Community Center Board,
several Bolivar Alderman, and the
Mayor, SBU alumnus Bill Jones,
had decided that I would no longer be able to receive contributions
through the Center.
This change in policy (which applied to me only) was prompted,
Mrs. Payne contented, by my friends'
attempt to organize a benefit
event through several local churches to help pay for my medical
expenses.
In order to reduce confusion, friends had arranged for the donations
to be given through the Center. Mrs. Payne
explained that Center and
the City of Bolivar (which administers the City of Bolivar's
"Delarue
fund," the Center's main source of funding) would not cooperate
in
any form of fund raising for me and did not wish to help me
obtain
medical care.
When the churches were recontacted to complete arrangements
for
the benefit, the churches, too, declined to participate or assist
in any
way on the grounds that, according to Mrs.
Payne, I was not worthy
of assistance.
Since the 1988 poisoning, despite numerous attempts, I have
never,
to my knowledge, received a penny of assistance from either
the
Bolivar Ministerial Alliance, or any church or church-related
organization.

I received a monthly payment of $86 during this time for my
Supplementary Security Income payment.
This is the amount I received after my Medicare premium was
deducted from April 1993 through December 1993 at a rate of
$329.40 per annum; January 1994 though December 1994 at a
rate of $469.20 per annum; January 1995 through November
1995 at a rate of $474.41 per annum.
Medicare, however, does not cover most of the treatment and
care which I require, and I could not afford the twenty per
cent
out-of-pocket expense necessary for those few covered procedures
which might have benefited me.
Additionally, I was not able to locate a Medicare hospital
facility
which was experienced in the treatment of such injuries and
which
was able to accommodate the precautions necessary for me to
receive treatment without further jeopardizing and compromising
my health and well-being; therefore, I was never able to use
my
Medicare coverage and received no Medicare benefits.
Due to the abuses by city, county, and state officials and concerns
over the involvement of the law firm of
Douglas, Lynch, Haun,
and Kirksey, which acts as legal counsel for the City
of Bolivar,
the County of Polk, Empire District Electric Company, Citizen's
Memorial Hospital (Kerry Douglas,
CEO of CMH), Southwest
Baptist University, and the Southern Baptist Convention
(Verna
Haun, Board of Trustees of SBC) and the tacit and blatant
threat
that any further attempt to obtain assistance would result in
another
forced entry and assault, I did not renew my application for
Social
Security Income and Medicare premiums and have not received
any assistance since November 1995.
